They're doing chest Compressions, CPR.
If we can bring him back, he'll Be urgently taken to the Operating room, we'll have to Wait and see.

I've always been in the front Lines.
I was an EMT, I was a Firefighter, and I'm in the army Reserve, the medical corps.
Found by ems awake, alert, Oriented, complaining of chest, Abdominal, and pelvic pain, Developed a slight an in Particular rhythm with our Pulses.
Any bleeding that you see? Throughout his abdomen but no Other obvious bleeding.
See if there's a pulse.
Is there a pulse? No pulse.
Continue CPR? Blood.
Can we get blood? I put myself through college And medical school.
My loans are almost up to $300,000.
First unit of blood is going In.
Continue CPR.
I would do it again in a Second.
I would volunteer to do this.
Stop and see what the rhythm Is.

We open his chest to try to Give him the best chance of Getting him down to the Operating room.

I know all about it.
When I turned 50, my primary Physician said you better get a Colonoscopy, which I should have Had done and I never did.
I just pushed it aside you know For one reason or another.
I wasn't real anxious to do it, Not just because I just didn't Take the time.
The good news here is that if You have to have a cancer, colon Cancer is not a bad one to have If we catch it early.
The treatment for it is surgery And we want to take out the Section of the colon that has The tumor.
We want to put you back together And hopefully find that we don't Even need to do anything like Chemotherapy at all.
I'm a former firefighter and For 28 and a half years I was Breathing some pretty bad smoke.
I figured well, someday I might Just get cancer, ya know? All right, we're going down.
My patient is practically Dying in front of me.
He needs an operation at this Very moment, that's his only Chance.
The heart's beating, there's No pulse without compression but There's a pulse with Compression.
All right.

Her name is Caitlyn Macphee.
Is she in any pain? No, I gave her some Medication that takes away that Pain.

She's got blood in her brain and They don't know why.
This little girl has gone Through a lot.
She went into renal failure late November.
She's on dialysis at home and She was doing really well and This afternoon she took a nap And woke up and couldn't feel The right side of her body.
So, me and my husband rushed Here, and what do ya know she's Got blood in her brain.
Okay.
The elevator is broken so Unfortunately we have to get Onto street level and then go Across.
Okay.
Is this the only way to get to The E.

Fung had some imaging That revealed that his Gallbladder may have been Acutely infected and inflamed And so he's on the add-on Schedule for the O.

Today to Have that removed Laparoscopically.
I've been on a morphine trip For awhile, which is good.
It's making you feel pretty Good? Much better, yeah.
I'm actually really looking Forward to it.
He's has a lot on the line here In addition to this person's Gallbladder.
Instead of buying you one Beer at the mission, I'll buy You two beers.
I need like a steak from Morton's.
That's what I need.
My bet with Andrew is to see If he could remove the Gallbladder so cleanly that bile Doesn't leak from the Gallbladder, which will force us To use the device called the Suction irrigator.
No, no, no, trust me I got This gallbladder operation.

Now she's got blood in her Brain.
Caitlyn Macphee, she's 4 Years old.
Today around 6:00 P.

She Started complaining of a Headache on the left side, right Side of weakness, was brought Into the hospital by her Parents.
We're gonna make sure that All her vital signs and Everything are okay.
Most likely depending on what we See on the ct scan, we're gonna Repeat the ct scan to see if the Bleeding's changed at all.
Okay.
And we'll have the Neurosurgeons come down and look At her also.
They're the doctors that Specialize in that area and She'll probably go up to the Intensive care unit.
Okay.
There's only so much room Inside your head, if you have Some blood occupying that space, You end up pushing against parts Of the brain so that they don't Do what they're supposed to do.
That's in fact what's happened To her.
That's why she's not moving her Right arm they way that she's Supposed to.
She's moving her left arm Spontaneously and her right and Left leg.
She's doing just fine, mom.
She's moving around a bit, That's perfectly fine.
We want to see if she's able to Move, that's actually helpful For us.
How we doing, okay? Be my brave girl, okay? Be my brave girl.
As soon as the neurological Assessment is done we're gonna Go across the hall to the cat Scanner to see if there's been Any change to her bleed, okay? We're gonna take some Pictures of your head, okay? When you're doing a shift in The emergency department, you You definitely go home and give Your kids a big hug.
The good news is most of the Things we see, kids get better.
We're about to take Somebody's gallbladder out.
Allow me to introduce myself, I'm Flavio.

My team is my chief resident, Flavio, and my intern on the Service, Andrew, also known as Bardouche to the team.
What's the deal? Amy, would you like to inform Flavio of our arrangements? When I used to do Gallbladders as a young Resident, my attending used to Put a little incentive.
If they did have to bring out The suction irrigator then they Would buy like taco bell, right? So young Andrew here says that He doesn't think that he's going To need to do that.

He does talk a lot of smack, But we'll see how he does on This case.

There's a lot of inflammation, a Lot of water in the wall, which Should make the laparoscopic Approach a bit more challenging.
This is pretty tough.
Can you introduce Andrew to His hands? I've never used this Instrument before! Do you want me to just tee it Up for him? Flavio what is this tee ball? Flavio has two objectives.
To eat steak and to make my life Hell.
Andrew, it's my job to train You.
You start internship and they Tear you down, until you almost Feel completely naked.
I mean, that's their goal is to Make you a completely different Person.
If you can make it through Internship, if you can make it Through residency and Fellowship.
I mean those are the people you Want operating on you, the People that were able to Withstand that.
Now, here's the question you Have to ask yourself, is there Any chance this could be the Common bile duct? No.
Why is that? Here's the gallbladder and This is the duct going right to It, going nowhere else.
So it's not double-backing Into the liver.

The wall of the gallbladder was So inflamed that it started Leaking some bile.
We're gonna irrigate.

I think we were able to get The gallbladder down safely, That's the most important thing.
Um, and I think the patient will Be fine.
He can go home tomorrow, no Problems.

My chief was just as hard on Me as I was on him and I think It's my duty to do that, but I Think in the end he's going to Develop into a good surgeon.

And all of a sudden he was Confronted with the fact that he Had a colon cancer.
That's a nice one.

Judy came into my life when I Wasn't doing too well.
I had a divorce after 38 years Of being married and it was Very, very difficult.
Extremely difficult.
This island is a great place To heal.
When I came down with this Illness, Judy was right on top Of it.
I kind of knew that there was Something serious before Anything came back.
I just had a feeling.
I don't know why, but I just Did.
But I didn't say anything I Didn't want to be alarming.
They wanted to get it done.
The worst thing about something Is leading up to it.
They say you got a malignant Tumor and it's not spreading, so They say.
You really don't know until you Get in there, do ya, really? nice deep breaths, okay? Give me a deep, deep, deep, Deep, deep breath in.

Alden is confronted with Cancer.
That's tough to take for all of Us and Mr.
Alden in particular.

Then something happens that Makes you think you're a little More destructible than maybe you Thought you were.
Okay, so the tumor looks to be Right there.
Okay, let's make a little hole Right there.
I met Tom last August and we Just hit it off.
All we do is laugh all the time.
He's a gentleman, he's just Wonderful.
I finally find the right man, And I'm hoping he's not gonna Leave the earth, you know? Tie those.
Okay, so the reason that we do Colonoscopies is to pick up Colon tumors.
And you'll be able to see quite Easily the difference in this Flat mucosa versus this cancer.
So the next step in his Treatment is to wait for the Pathologist to look at it under The microscope, tell us if it Went through the wall of the Colon, take a look at the lymph Nodes and see if there's any Tumor in the lymph nodes.
His chance of survival is gonna Depend on the results of those Tests.
She's talking and not on Machines.
She looks ten times better than When she came in.
She's smiling, she's playing.
She's had a bleed in the brain, The bleeding has stopped and Some of the blood has gone down Which is she is starting to get Some of her functions back.
She's a fighter though.

Caitlyn is getting an MRI of Her abdomen.
She's been having upper left Quadrant pain, so they're trying To figure out what's causing it.
She thinks that we're gonna Do sedation, but we're not.
We're just doing tube in and Then fluid.
Okay.
Because of the type of test That she's having done, she is Definitely going to be Intubated, so just to let you Know.
Are you serious? Why? Because we have to give her Some fluid for the test.
They didn't tell me that Though, when the Anesthesiologist came they said They were gonna give her iv Fluids.
So we're gonna go through This all over again? They shouldn't have told me One thing and then when I come Down here they tell me another Thing, 'cause that just isn't Cool.
She's gonna be miserable Again.
I don't want anything done Until I speak with the Anesthesiologist and know Exactly what's happening to my Child.
The mom is very, very upset.
Apparently whoever went and Prepared them for anesthesia on Friday told her that she wasn't Going to be intubated.
Drama, drama at children's.
I'm on my way now to see one Of my patients that I'm taking To the O.

He's fine.

She's a 21-year-old who had a Cesarean section ten days ago.
Unfortunately, she developed a Severe infection.
My stitches had given out so When I was in the bathroom I Just started bleeding all over The floor.
Today I'm going to do my best To make sure it's all cleaned Out so that she could go home And take care of her baby.

I'm certain that if I had more Time I would go out and meet People and you know, fall in Love and get married and have Children.

So you can imagine having a Child as sick as her child is And then all of a sudden Expecting one thing and then you Know ten minutes before we're Ready to do the scan you have a Whole different picture.
I'm sorry, I'm still paying Hundreds and hundreds of Thousands of dollars for this.

I'm one of the attending Anesthesiologists.
I apologize for the confusion Today, but Caitlyn was put on The schedule in order to take a Look at a specific area of the Abdomen to try and identify what Is causing her pain.
In order to get good pictures, She can't breathe.
We stop her breathing during Periods of the scan.
We weren't even told that.
People should've come talk to Us, you know, we could've had Time to research.
We just sit around the room and Get nothing done, but the kid's In pain.
We get down her 20 minutes Before the procedure and we get Like hit with like a blow of Information.
The communication is Nonexistent in this hospital, And I don't understand that.
I really it's bad.
You've got four or five Different modules within the Hospital trying to come up with A plan and it just didn't get Communicated well.
There's no reason for you not to Know what's going on with Caitlyn.
It's not fair to caitlyn.
It's not fair to you, but at the End of the day, the next few Days, either today or the next Few days, we'll be doing a Study.
I'm gonna let you guys talk About it.
If it was more of a Life-threatening situation I Would say you know what we Really need to go.
We're not there, she's fine, She's stable, and she's got some Pain issues.
You know, just want to make sure That you're comfortable with Everything.

they were trying to figure Out what was wrong with her.
They were trying to make her Well, but now it's like they Don't even know what to do Anymore.
It's just like "why am I here?" Caitlyn's been in the Hospital for six weeks.
The next step is to get her Home.
So we've done all the studies, We're not gonna do anything new.
Her pain is under control.
Everything is moving in the Right direction.
Mom has learned how to do Dialysis at home so the kid Can get the treatment she needs.
Dr.
Harmon thinks she would Benefit from extra dialysis.
That was something that people Are feeling pretty intense about Right now.
I know she needs the Dialysis, a minimum of ten Hours, but to get her on for 12 hours now is gonna be even More challenging.
She's gonna get dialysis Everyday, she gonna get a dozen Medications, she's gonna get Some injections, she's gonna get Physical therapy.
Think about yourself.
How would you handle all these Things? So this is a very daunting task.
I'm responsible for so Freaking much dialysis, Rehab, doctor's appointments.
I set the machine up, I get the Doctor's appointments, I haven't Seen my son in a month, I can Barely get her on for ten hours, They don't even know if this is Gonna work, but my life's gonna Get turned upside down.
It's a huge, huge, huge Undertaking, but if there's one Person on this planet that could Do it, it is you, because you Are fantastic when it comes to Taking care of your daughter.
It's like, I can't breathe And I'm trying to hold it Together because it's not her Fault.
The anger is very real and Very valid.
When you're sitting there, There's not much you can do as a Caregiver besides just trying to Absorb a little bit of it and to Not to take it personally in Any way, shape or form and to Hope that it in some way helps Her move forward.
It's very, very normal to Feel like you do.
You do so much and you do it Well.
You do it very well so that is Why you are going to be able to Handle bringing her home and Doing it again.

If the cancer has spread to the Lymph nodes, his tumor is gonna Much harder to beat.

Alden, and Unfortunately, he has three to Fourteen nodes positive for Cancer and because of that he's Gonna need chemotherapy.

I'll ask the doctor's advice.
So this isn't the best news, And it's definitely not the news That he wanted to hear.

How are you? So the pathology report is Back and this cancer did not go All the way through the wall, Which is good news.
But 3 of the 14 nodes were Positive.
Yeah.
For tumor.
Okay? Uh-huh.
So what that means is that it Spread into those lymph nodes, Right near the tumor all right? Hopefully we got that out.
It does translate though into Most likely needing Chemotherapy.
Really? Wow, okay.
Obviously nobody wants to Hear that.
No.
Nor do I want to tell you That.
But I think you have to look at How this thing has gone, you Know, you had an operation.
The operation went really well.
You're doing really well and you Jumped the first hurdle and the Tumor is out.
And now what we need to do is do The second hurdle, which is do The belt and the suspenders, Make sure our pants don't fall Down, right? You know, I really like being a Surgeon, I help a lot of people, I have fun doing it, but Ultimately the thing that will Make me stop is the Complications, is the problems, Is the bad outcomes as Ultimately that will drive me Crazy.
This sucks.
I just arrived in Cairo after A 12-hour flight.
Karem is my cousin and we've Gotten pretty close and neither Of us speak Arabic, so we're Totally screwed.
